Who is Eligable?
People diagnosed with life-limiting illness—including but not limited to cardiac disease, dementia, cancer, kidney disease, neurological or pulmonary disease.
Consider these questions:
- Does your loved one have uncontrolled pain or increased shortness of breath?
- Does he or she require assistance with routine daily activities?
- Has there been weight loss, loss of appetite or difficulty swallowing?
- Have there been frequent visits to the hospital or doctor’s office?
- Is most of their day spent in a chair or bed?
- Have you shifted focus from treatment to providing comfort and enhancing quality of life?
- Is the disease progression nearing its final stages?
- Have they been getting frequent infections, or are they incontinent?
- Has he or she been falling frequently?
