Halcyon Hospice is pleased to announce grants available to rural hospice programs. Get the Flyer![]()
Rural will be defined as a (HPSA) or a Health Professional Shortage Area.
Halcyon will be making funds available to rural hospice programs who have families in need of transportation and lodging assistance at crucial end-of-life moments (to be determined by the referring hospice). Funds availability will be by budget per quarter. Eligibility will be determined by the referring hospice. The referring hospice will have made a contribution of 15% toward the request. The Foundation reserves the right to determine awards. Please contact Halcyon Hospice with questions and reference Community Service Grants.
